e. Drug Test Result from RFU1 (Available at the venue)
2) NEW LTOPF:
a. Filled-up and notarized LTOPF Form (Form available at the venue)
b. National Police Clearance
c. Affidavit of Undertaking for 10 year validity period of LTOPF (Available at the venue)
d. Neuro-Psychiatric Clearance (Available at the venue)
e. Drug Test Result from RFU1 (Available at the venue)
f. PSA/LCR Birth Certificate or Photocopy of Valid Passport
g. Gun Safety Seminar Certificate (Available at the venue)
h. Proof of Billing or Brgy Clearance
i. Photocopy of Two (2) Valid Government issued IDs
j. Proof of Income- Certificate of Employment/ Appointment Order/ Oath of Office/Land Title/Certificate of Bank Deposit/Business Permit
